My feeling is you are trying to be correct when you should be as incorrect as possible.
Why Waapas ab us duniya?- I know people talk like this now- as though mangling own mother tongue is a proof that one is 'educated' 'secular' and so on- but, even in English poetry, people have to get rid of the 'Bureaucratic-Technocratic' mindset and write in more ambiguous, but also more idiomatic terms.
Why so many pronouns? Abh tak tera khwaab aankhon pe hai (you are not doing an Exam) Once you write like this you will yourself see that the second line is not worthy. Kare sab kuchh hamen qurban to kaise?
Poetry has to be interesting- things sacrifice you, you don't sacrifice things unless you're writing a letter to your Mum or something.
If the other person is tum how are you aap? You use hum, because- in this context you are Everyman.
What is the connection between having her dream in the eyes and not knowing how to leave all you have?
Why not 'us duniya wapas jaaun to kaise'
Why say- main kuchh bhi sochoon? why not 'khair kuch bhi sochun aa jaate ho tu khayalon mein/ Is jalal-o-jamal jaal suljaaun to kaise.
In Ghazal jalal and jamal are two aspects of God- viz horrendous power to punish and all Merciful Beauty.

Better than bachaaun is bichaaun. You don't want to save your days and nights from her Sun and Stars. You just don't know how to roll out your mat of nights and days so as to remain under her gaze.
The same 'salt & pepper' is needed- to my taste in every line. Why 'zindaa' when the word zindan is more poetic. Tu aaye to aaya yaad hum zindaan main hai/Ab zindagi hai zikr bhulaaun to kaise?

Just recently, I've begun to read about Al Rummani's theory of tadmin (tazmin) and to recall- ijaz- brevity gains balagha (eloquence) when there is an irregular preposition for tadmin words (i.e. those with implicative meaning).

One must forget grammar- which is fine for bureaucratic/technical tasks- when it comes to poetry. If you write zindagi- it may be your real meaning is zindaangi- you have to break your bureaucratic mindset before you know what you yourself mean.

This is lovely and has many gems. I think, in poetry, elision-i.e lexical ellipsis (ta’qid-e-lafzi- adds beauty so you can omit double 'se' and write
Hun bewafaa jaise duniya jab se
which is more poetic because it has both meanings.
Similarly
Aaghaaz-e-ashaar hua tum se jab se is too plain and Hallmark card.
I would instinctively write 'tujh bil jabr se' because there is more tazmin for such a thought.
Otherwise, the meaning is rather slack. Writing everything in the same ink is not poetic- we do that anyway. Writing everything in the dark blood of the disgrace, black as ink, of deflowerment, however has a different meaning.
Recently, a visitor to my blog drew my attention to the relationship between the sakshi (witness) of the Upanishads and the Shahid (martyr means witness) of the Abrahamic traditions. Sarfaroshi ki tammana- zeal for giving one's head (but it must be a virgin offering!) is common to both popular Hinduism of all regions of country as well as Islam.
I am judging you to be a younger man than myself- so forgive my senile remarks.
Great poem. Thousands can be expert in a language- few can write genuine poetry in it. You can. God bless.

Lovely poem- but difficult for me to read as without diacritics. Why not put a real audio link?
Of course, younger people, of the texting generation, may not have a problem. Still, with a good poem it is worthwhile to put it up in both Hindi and Urdu font- which I believe is now quite easy.

The poetic pivot, for me, is 'Dono ke aks hain wahaan
Tar hain jheelen phir kyun aaj'but I don't know if I've understood correctly.
On the other hand- Meraa hi chaman hai inkaa ghar
Kyun hijraton ko jaate hain
Inki ek ek chehchahaahat
Mausiqi ban ke goonjti rehti hai yahan kab tak..- which is a very poetic sentiment- you may not be aware of the story of the 'ababeel' (little birds which saved the Kaba from Abraha)-but, it seems to me, this lovely passage shows a sort of English idiom beneath it(hijraton ko rather than hijraton pe)-this is quite fashionable (like Kolaveri di!) but since you are fluent in English you are missing an opportunity- put the thing in a fully Rekhta idiom- like you're an actor doing improv who doesn't know any English- and I think the thought will become more beautiful in your own mind. It is a Hindvi thought. In fact, this is the excellence of the poem- understated, with some winsome childishness balanced by nicely judged, wry, self reflective wit.
A similar point can be made w.r.t this lovely 'mazmun'- Zulfon ke ghane andhere behte dariyaa
Kho jaane ko
Jabeenon ke maahtaab
Raah dikhaane ko

Jabeenon is just plain wrong unless the idea is that there are many foreheads but the moon of that forehead always remains full- i.e. the poet is so naive that he thinks that different coiffures of the beloved- which obscure parts or all of the forehead- are like different skies above different planets. This is a good 'Ma'ni afrini' but you can only fully potentiate it by going more deeply into yourself so you are within a purely Hindvi womb and English syntax can't work its barbarous will upon the beauty of the thought.

It was indeed satisfying to write...though interestingly, the effort required happens mostly in the subconscious, and from there it's just a question of letting go one's imagination and it flows out in one natural stream of consciousness.
Great point about line breaks- I think for me they fall in place just as I imagine reciting it aloud...
